I am using lvm2, and have my /usr, /home, /var, /opt on lvm2 volumes. However using 'BAD_INSSERV_HACKER=true dpkg-reconfigure insserv' lvm2 ends up at the end of the list in /etc/rcS.d (S26), and checkfs is S08. Booting fails. I solved this by doing cp /usr/share/insserv/overrides/lvm /usr/share/insserv/overrides/lvm2 Now lvm2 is S06, and the booting succeeds. Please consider adding an override for lvm2 too.
