It's mostly a question of how you want to surprise the user. If you only save modified preferences, then when the user has specifically set an option to something that happens to be the default, or when the default is changed to match their preferences, changing the default will suddenly change a preference the user explicitly selected. aptitude chooses to be conservative and save any preference the user has ever modified from the interactive options screen, even if they modified it to the default value.
