I think I found the problem:

the saving works fine, but it takes quite a while. The dialog
showing the progress doesn't appear anywhere close to the gscan2pdf
window, but flashes up in the top-left corner of the left-most head
in my triple-head setup. Since I run gscan2pdf on the right-most
head, I never noticed it.

So I hit save, and the file was created, but not noticing the
dialog, I assumed it was done and had failed to write the file
contents.

Since I can quit gscan2pdf without problems or requiring
reconfirmation while a save operation is in progress, I left the
programme before it was done writing, thus losing the data.

I see two things that should be done to address this:

- make sure the progress dialog appears normally
- make sure that the application can't be quit, or at least requires
  confirmation when quitting, if a save is in progress. Maybe all
  that needs to be done is postponing the clearing of the dirty bit
  until the save is complete.
