Which program are you talking about?  If it's caff, then you can already
do it.

caff basically calls gpg's --edit dialog with the sign command.  abort
that sign (saying no to the 'really want to sign' question) and run
issue 'showphoto'.

If you're unhappy with the image just select the other UIDs (those that
you want to sign) and issue sign, do the dance, and then save.
