Hello Andreas, The real problem with creating "canned" packages for VistA is the sheer complexity of the application suite. The VA's documentation is good but assumes that a good deal of knowledge about the product exists.
I personally have not tried to build a legal Debian package so I don't really know what is involved, but I not really sure that VistA is usable without work on the user's part. Installing the product is the least of your worries. The problem is that I'm personally completely unable to do any useful stuff with Vista even if I would get it packaged right into a Debian package. We certainly need somebody who is able to test and verify functionality. Building proper Debian packages would take the workload for proper installation from system administrators and using Debian-Med would bring a really free operating system for medical care which might be appropriate for the environment you mentioned.
