What about:
package (version) bla
 * Maintainer Name:
   - Done this
   - Done that
 * Another Maintainer:
   - Done another thing.

  -- Uploading Maintainer <[EMAIL PROTECTED]>  Upload date

Thats imo the best way: Everyone knows who did changes, and you see
who uploaded the package.
