Try adding this option:
Option "ZAxisMapping" "4 5"
and you may need to change your protocol to:
Option "Protocol" "IMPS/2"
I haven't tried any other protocols for wheeled mice. However, I can tell
you that imwheel is _not_ needed with XFree86 v4.x
