Look at the pppconfig package (to find it's full path, grep for it in /var/lib/dpkg/available).
It has a configuration that pretty much should get your manual modem connection working - assuming that the OS can communicate with your modem just fine (ie, your certain your modem is at /dev/ttyS1 or some similar com port).
