I have fixed the problem.  In the following section:

Section "Device"
        Identifier      "STB Velocity 128"
        Driver          "nv"
        Option          "UseFBDev"              "true"
EndSection

I used debconf to remove the "UseFBDev" option, and everything came up
fine.
