dpkg-reconfigure xserver-xorg -plow then restart X [Ctrl-Alt-Backspace when xdm/gdm/kdm is running usually works for me] Sometimes KVM monitor switches can "lose" a mouse temporarily - you may find switching to XP and back will work to reset the mouse.
