Followup for those scanning the archives: I had an off-list reply as follows: Make sure this is in order: http://www200.pair.com/mecham/spam/clamav-amavisd-new.html If clamav or amavis-new have been upgraded, the config files may have changed. This howto was useful in identifying the problem, but didn't address my particular issue. In fact, I changed permissions on /var/run/clamav from 755 to 775, and restarted clamav-daemon successfully.
