Maybe change WARN action to put:
X-RBL-Warning: TESTNAME "DNS test returned txt record"
... or to use the example:
X-RBL-Warning: ORBZIN: Open relay. Please see http://orbz.org/?165.252.48.3
X-RBL-Warning: ORBZOUT: Open relay. Please see http://orbz.org/?165.252.48.3

You have given us a facility to list on one line all of the tests that
failed with the %TESTSFAILED% thing, but this could be a cleaner way to put
them both together. Gives us more detailed info for debugging, along with
keeping the number of header lines added to a minimum.

Just a suggestion. Keep up the good work Scott!
