The E-mail failed the ORBZIN, ORBZOUT, OSRELAY, SPAMCOP, and ROUTING 
tests.  That combination of tests also set off the WEIGHT10, WEIGHT15, and 
WEIGHT20 tests.

The ORBZIN, ORBZOUT, OSRELAY, and ROUTING tests are set to LOG, so no other 
action was taken on those tests.

The SPAMCOP test is the only one of those set to the WARN action, so that's 
the only warning you will see in the headers.

The WEIGHT10 and WEIGHT15 tests are set to the SUBJECT action, so the 
subject was changed.

The WEIGHT20 test was set to hold, so the E-mail was held.

>How come the Bounce messages did not get sent?

Because you have the WEIGHT20 test set to the HOLD action.  That's why the 
E-mail was held instead of bounced.

>Why does it look like each test gets run twice?

In this case, there were multiple recipients, causing the tests to get 
listed twice.
