That will occur if there is a Received: header that contains a bogus IP 
address.  In this case, it appears that the Received: header contained "[]" 
in it, which isn't valid (it should have an IP address within the brackets).

If this is just happening on a few E-mails, it may not be a problem.  If it 
is happening for all E-mails, then it would indicate a problem on your end.
