I usually only see this when it is caught in my spam folder.  Does this
type of warning ever appear in legit mail?

Yes, this often appears in legitimate E-mail. You just won't see it, because it appears in the MIME preamble, which few (if any) mail clients display. If you open an .mbx file on your IMail server that has legitimate E-mail in it, you will likely see that text (or similar text) in a number of the E-mails.
