Well, I'd never heard of or tried REALRECIPIENTS, so I can't comment. The notes from the release notes:
Added %REALRECIPS% variable that lists actual recipients (as opposed to %ALLRECIPS%, that lists intended recipients). It is listed as ALL so it applies to Virus and Junkmail.
