That was my first thought as well, but I realized it was biased. Having a
"dedicated department" could mean both manual review, and developers
creating rules to eliminate spam. I can't imagine a software company not
doing some of that.
I think there are people on this list who have probably gotten to the 99.9+%
level with the help of Declude, Message Sniffer, Spam Assassin, and the
members of this list. I'm betting Microsoft is coming up with similar
technologies and is using their accounts as a testing ground. There would
still be a manual review process to filter out the email that needs or
deserves a response, as with any high-level exec, but I can't imagine that
they don't have some sort of rules processing engine behind the bulk of the
process.

Ah, they at least give a hint as to what they are using:

Given that Ballmer presumably receives in the order of 100,000s of spams a
day, that means a 99.99+% spam capture ratio. That pretty much means that
he is using the same Microsoft anti-spam technology as Bill
Gates: "practically an entire department at the company he founded is
dedicated to ensuring that nothing unwanted gets into his inbox."
So Microsoft's anti-spam technology is about as low-tech as it can
get. It's a bunch of people that go through millions of E-mails each day.
