How many files do you have in your spool? Anytime I've had this problem there has been a corresponding increase in the spool contents.
I agree with Matt that it may well be a user sending high volumes of SMTP traffic through your server. It doesn't have to be a lot of messages, but if they are very large that can cause problems. We have a bunch of employment and temp agencies, and they cause us trouble sometimes because they tend to send large attachments.
Also look for a dictionary attack against a domain with a nobody alias.
