Also, My impression is that this may be an authentication issue. How can we get a username and password into that connection string?

Traditionally,  this would take the form ...cn=users,dc=example,dc=com
--  not  ...cn=users,dc=<domain  controller>.  Are  you sure this is a
valid LDAP context in your setup?
