Yet another thing can happen when debugging a dll under XP (Win2k was fine):

If the dll resides in a different dir than the source (common for isapi
dlls), XP loses the module path and debugging won't work until this is reset
by going ctrl-alt-M, finding the dll instance, changing the path and
reloading.
