I belive it's a good habit to use a new name for the patch file when
updating it in Jira, including a version number or a date e.g.
derby-819_3.diff or derby-819_20060124.diff.

This just reduces the chance that a committer will commit the wrong
patch, by accidently using an older version that was already on their
system.

With this approach there is also no need to remove the old patch, they
can be useful sometimes, to compare the aprroaches.
