New version of specification looks much better, but still has some problems.
1. There are too many typos and they sometimes affect the meaning, e.g.
"null,null,1 null.null,1 no exception" - what is "null.null" (dot instead
of comma)
"null,1,null null,null,1 no exception" - likely should be ""null,1,null
null,1,null no exception"
2. After "no exception" shall be " and table contains both inserted rows"
(otherwise it is legal to assume that second row may be silently swallowed)
3. Issue is named 'Add "Unique where not null" to create index', but discussion
of unique indexes is completely absent from functional specification.
