The one time when I saw this was when I tried out Chandler Hub. (Normally I use the Desktop client exclusively.) I filed bug 11413 on this, but no progress so far. Maybe you could keep an eye out on whether the items that pop up to NOW show as edited and whether that person uses the Hub.
