I've seen some desktop deployments (home on NFS) that once used it, but mainly because it could be done, not because it was absolutely necessary. However, in an utterly different use case, I have seen fam used with the Courier IMAP server, between IMAP frontends and the file storage backend. It's used for client notification, so almost as soon as an email is dropped into a Maildir by the MTA, it's visible to the client.
