Exactly-- the essence of the HIG guideline is just that the most distinguishing information be shown first. In most cases, that /is/ a document title or filename rather than the application name, but I'd certainly consider it to be in the spirit of the HIG guideline for a particular application to do something different (but sensible!) to make its windows easier to tell apart.
