Title:
Multiple icons on launcher for the same application, number hints
  always shown

Status in “unity” package in Ubuntu:
  Fix Released

Bug description:
After using my desktop for a day or so unity gets in a state where I
get multiple icons on the launcher for some applications and the
number hints (normally only visible when you hold the super key) are
always shown. I'm attaching a screenshot.

The number hints are only shown on the launcher for one of my
monitors. The duplicate icons appear on both.
