The bug report had expired, hence I reopened it. Yes, the issue is
indeed still present in 12.04: occasionally unity simply "forgets" about
a particular window and it does neither show up in the alt-tab not the
alt-' and are also not listed in the launcher. I found no way of getting
the window back, not even by killing compiz. I'll provide more symptoms
if it occurs again.

Bug description:
When I have more than one instance of a program open, after minimizing
all windows, clicking on the icon in the Unity launcher opens only one
window.
For example, if I have two Chromium windows open and minimize both,
one will become 'lost'. If I click on the icon in the Unity launcher,
one window will maximize. All of the little white triangles under the
icon which indicate multiple instances open remain, but I can only
access one window.
Alt + ` only reveals one window.
Alt + tab just shows the single icon.
If I then minimize the single window which has maximized and then
click on the icon again, the other window will maximize.
What I expect to happen: clicking on the icon in the Unity launcher should
show ALL windows in a rectangular grid on the screen. Alt + ` should let me
scroll through the open windows in the same grid.
I am using the latest compiz-config package with Ubuntu 11.10 Oneiric.
