Turns out it is a keyboard wiring issue. For quite a number of keyboard models, Capslock + Shift + <key> don't work for a number of keys, confirmed by xev. I managed to find one keyboard where Capslock + Shift + W works sometime back, but don't recall the keyboard model though.
