Same issue here, only reboot seems to solve the problem.

  Sound through headphones randomly stops working

Status in “pulseaudio” package in Ubuntu:
  Confirmed

Bug description:
  Sometimes, apparently randomly, sound stops working through the
  headphones. Sound still works normally through speakers but not
  through headphones. Pulseaudio correctly senses the headphone
  plug/unplug and the device appears in the output devices list but no
  sound is heard.

  I've tried killing pulsaudio and restarting the session  but only a
  reboot solves the problem.
