The following worked for me. I am on 12.04 LTS. 1. If you have texlive-publishers installed, remove it. (This step is necessary as otherwise it won't let you do step 3).
2. Follow the steps in this link: http://ubuntuforums.org/showpost.php?p=9097651&postcount=4 3. Get and install the texlive-publishers package using this link: https://bugs.launchpad.net/ubuntu/+source/texlive- extra/+bug/575812/comments/2 This resolves the affiliation bug.
