pulseaudio dies when I start a song/video

Bug description:
Binary package hint: pulseaudio
when I start a song or a video and let it play without pausing it for
5secs pulseaudio crashes. *but* if I start the song and as soon as the
player comes up and I click on the pause button and wait for a few
seconds and then start the song it works fine. now if I click on the
next button and another song starts pulseaudio will crash too :(
