If I was to guess I would definitely go with option 3, "are we just regressing because of a lack of tests (we still have a huge technical debt to repay there)?"
Luckily last I talked to James Lal there were too many people looking to contribute to the integration test framework to actually deal with, it is scheduled to be usable shortly, and as far as I know all the fancy new functional teams (certainly the browser team) are switching their workflow towards requiring tests for for items to be complete.
