I've just put together a brief page of the sort of information we
currently need when reporting bugs relating to Loop calls failures
(during initiation, or during the call):

We've bugs on file to reduce error reporting, and allow automatic
submission of at least some information, so hopefully the list might
reduce as we move forward.
For now, getting the information on that page will give us more chance
of working out the issues and being able to fix them.
If anyone knows anything more we should be logging or if I've got things
wrong, please do update the wiki.
