I assume that this is temporary only and that you have a plan to have this automatically reported (maybe with user consent).
This isn't really a scalable solution; it requires quite a lot of effort on the part of the user. If all this process could be reduced to the press of an "I'm having issues" button, I'd be a whole lot happier.
