There seems to be plenty of issues:
ERR_04486_VALUE_ALREADY_EXISTS The value 'preferredDeliveryMethod'
already exists in the attribute (m-may)

ERR_04486_VALUE_ALREADY_EXISTS The value 'telephoneNumber' already
exists in the attribute (m-may)
ERR_04486_VALUE_ALREADY_EXISTS The value 'preferredDeliveryMethod'
already exists in the attribute (m-may)
FTR, I have created a OpenLDAP ITS for those 4 errors :

The ITS will be closed. These are not errors on OpenLDAP's part and we
are not the ones who can fix them.
