Nope. Slipped by in haste to get it out. The original is a spreadsheet
and required much manual intervention.

I think I have fixed it by using the following:

This document provides a cross-reference of the various Avery formats
that are equivalent and available from other manufacturers.

Oh thanks. 

Just a comment. I do wish that there was more effort in producing
content than in playing proof reader. In this case it was warranted as I
had guests and did not take the extra time needed to fully check. Don't
you, as members of the project, think that making the effort to create
content is worthwhile?  Puzzled.
