I have had that problem when putting lists into Notes, Tips, Cautions. IIRC, it was caused by a bug in Mediawiki. I thought it had been fixed in one of the wiki updates, but perhaps it has turned up again.
I will check some of the Notes in other chapters to see if they are also broken.
