I have had that problem when putting lists into Notes, Tips, Cautions.
IIRC, it was caused by a bug in Mediawiki. I thought it had been fixed
in one of the wiki updates, but perhaps it has turned up again.
I will check some of the Notes in other chapters to see if they are
also broken.

The Note code in other chapters seems to work fine with lists, so my
suggestion above about a bug seems to be wrong.
The code for the Notes in other chapters looks like this:

It uses a styled <div> and wiki table markup, not HTML table markup.
Is there some reason why that markup does not work for you?
I think your HTML markup needs <tr>....</tr> around the <td>s. I just
added those; see if it works now.
