Added a translation_pending header to (almost) all markdown source files to
indicate that there translation has to be manually completed. Once translation
has completed, the header should be removed.
This header serves two purposes:
(1) it makes it easy to find markdown source files which are still pending
cleanup
(2) if the header is present, the translation process will render a tip box at
the top of the page indicating that translation is pending. In addition a link
to the original Confluence export page is displayed to allow for the
translation process.
