Thank you for sharing the results. They are very good picture of the current status, in my opinion, the survey could be scheduled (e.g. quarterly) and then review the progress of each indicator based on the actions taken in the previous period. That will give us a view of the continuous improvement of each indicator.

We received 40 responses out of a community of 342 listserv subscribers, so ~12% response rate.
There are several ways in which these results can be read, and I would encourage us to not read too much into them.
but... If we assume that these are representative of the community i.e. without significant bias, then I think it generally shows a project trying to encourage contributions with some challenges. I think the feedback is that the project approachability can be improved in a number of places and ways.
