Sorry, but the error report seems to be very strange for ActiveX
control. I would suspect that ActiveX control is not used in this case
at all.
If ActiveX control is installed and activated, then in case of a problem
while opening the document in MSIE I would expect currently a kind of
message "API call exited abnormally". Most of the OOo error reports
should be disabled. Is document opening triggered by clicking to a link
in MSIE?

Could you please explain the scenario in details. How the file opening
is triggered ( klick to a link in MSIE, drag and drop to MSIE, OOo or MS
system FileOpen dialog and etc. )? Which protocol is used in the
link/URL ( http:, file: and etc. or just a system path to a file in the
file system )?
