mod_auth_ldap and mod_ldap were significantly refactored and many bugs were fixed between v2.0.x and v2.2.x. Backporting the fixes to v2.0.x wasn't practical at the time (they needed to be split between httpd and apr), so you'll probably find most of the issues you describe have already been fixed.
I recommend if you intend to use the mod_ldap stuff, use v2.2.x. Regards, Graham --
