yea - but what about users whose first language is not english but who want
to use your resource in english? They may find it difficult to respond with
the correct vocabulary.
Also - your second point about keeping a list of variations is also
problematic. I'm not sure you can second-guess a subjective response.
Keeping a captcha strictly logical (eg an alphanumeric response) a) enables
multilingual responses where english is not the first language and b)
prevents a subjective response.
I think the image token approach will frustrate users to the point of them
quitting the activity.

Rich, I take your language point onboard and would resolve that with a
short
list of selectable languages, but you only need to show languages that
your
site supports. If the content is all English then there's no point in
supporting other languages in the captcha.
As for confusion over a picture being a cow, bull, calf, animal or
udders(!!!), you could use ListFindNoCase("cow,bull,calf","#user_input#")
to
give users a small amount of scope. I would deny "animal" as a bot could
insert that all the time and pass against chicken, dog, cat, etc.
