Yes, CGI is a global scope that provides access to server variables.

And yes, Application is another global scope, that is available for an
individual application (but not other applications).
There are quite a few others, depending on context. Here's a page that
explains them:
http://livedocs.adobe.com/coldfusion/6/Developing_ColdFusion_MX_Applications_with_CFML/Variables7.htm

If you're using a framework (eg: Fusebox) there can be multiple entry/exit
points, but otherwise you've either got:
- Application.cfm/OnRequestStart.cfm entry/exit.
- Application.cfc component, which contains onApplicationStart/Stop,
onSessionStart/Stop, onRequestStart/Stop.

There are books available, but I'm not sure what to recommend; I'm not much
of a book-learnery person.
Other ways to learn is to just read mailing lists ( like this one, or
CF-Talk on houseoffusion.com ) and also blogs ( eg: fullasagoog.com,
feed-squirrel.com, weblogs.macromedia.com )
