Do you still have the Key Request file? dunno about IIS 5, but in IIS 4, it contains all the details you entered, plus the certificate request. And you should have backed these up, if you go by the recommended procedures (i think). Look for some .key files, the details can also be read there, and I daresay you could just use Key Manager to get all the details out anyway.
