When I say the "Project" I mean in the "Package Explorer" view the top level 
folders are the "Project" you can right click on it and select "Properties" and 
you get a project properties editor which has the class path, builders, etc.

The reason why I suggest putting it here is I think it might be easier to 
develop rather than a form.  Your call though.

You can start with something simple like

"Maven Project"

which will have the following fields:

groupId
artifactId
version

Keep it simple for now.  You can add additional entries later.

Also don't make one uber property sheet that has everything maven.  Split it 
into different components.

Perhaps one for builder etc.

I am not sure if the selector box can support tree like structures, but if it 
can you can group all maven related properties into one group.
