One place where I've seen this happen, is if you select a locale that
is a "sibling" locale of the default one for the system (I think
_think_ that is why it is happening).
For example, if I select en_UK (or rather "English (United Kingdom)")
on the demo server (where the default locale is en_US) in the Order
Manager I get a bunch of non-translated labels.

Jacques,
I spent some time looking at the "no fall-back" problem you keep
mentioning, and I can't duplicate the problem.
I commented out the French translation of PartyInvitation, changed
my language setting to French, cleared the cache and the label fell
back to en.
When I uncomment the French translation of PartyInvitation (and
changed the wording to make it different than English), cleared the
cache and displayed the page, the French translation appeared.
I repeated the steps several times and it always worked as expected.
