Just following up on this since it fell off the radar... I think we more or 
less came to a consensus to add the "Awaiting Testing Verication" state as I 
describe below.  

@Michael--should I enter a ticket for this?  If so in ITSM, or TODOS, or 
somewhere else?

Further QA Needed - with the following transitions:

Rework Needed (Assign issue) -> In Progress

Verify and Close (No screen) -> Closed

Cancel Issue (Close issue with reason and fixVersion) -> Closed


Then add a new transition to the Approved state:

Request further QA (No screen) -> Further QA Needed

And the same new transition to the  Code Review (Post-Commit):

Request further QA (No screen) -> Further QA Needed

Yeah, that won't be a problem. Trying to be short & succinct as possible while 
still carrying the meaning.
