Hello, Jon Burgess came up with the solution on IRC - the ordering of layers in the osm.xml file was wrong, and I needed to reduce the extents.
I moved the "coast-poly" layer above the three contours layers and changed the extent parameters to read "-179,-85,179,85". I have noticed some data corruption to the east of the summit of Ben Nevis there is a vertical stripe of weird artefacts. This can been seen on the Cycle Map: http://www.opencyclemap.org/?zoom=13&lat=56.80244&lon=-5.01835&layers=B000 It seems to stretch for a long way north to south.
