@jmarantz @Lofesa Hi guys, i have a small update for you on this matter. So, We moved to a memcached cache and all was good for a few months except in the last few days the 404 issue re-occurred on one of the sites. I noticed again that flushing the pagespeed cache helped initially but the same images would eventaully get re-written to 404s. The cache DB has plenty of free space. I noticed the only thing which seems to have fixed it was going into the server and deleting the pagespeed cache folder and re-creating it again with 777 permissions.
